Policy Change Request

When should you notify us of changes to your practice?

  • Contact Information Changes (address, phone, email, etc..)
  • Adding/deleting extenders and physicians
  • Modification of specialty
  • Modification of working hours

You can make changes to your policy with a quick e-mail or fax to our Customer Service Representative. In your request please indicate your policy number or the name on the policy, date of the change, and a brief description of the change.

Note: If a new physician or extender is joining your practice, you will need to fill out an application. Contact:
